This hub is committed to promoting high quality, patient-centred care with all health services working together in providing fully integrated care for the patient. HHDW offers a variety of patient-centred services that will be a mixture of bulk-billed and private. What is Patient Information?

For each patient we have an individual patient health record (paper, electronic, or a combination of both) containing all clinical information held by our practice relating to that patient.

Patient information is anything that includes medical details, family information, name, address, employment and other demographic data, past medical and social history, current health issues and future medical care, Medicare number, accounts details and any health information such as a medical or personal opinion about a person’s health, disability or health status.

How do we collect patient information?

We collect your information when you complete a new patient form, when we update your details over the phone, when you book an appointment online, or when a doctor updates your individual patient file during a consultation.

In other instances, this information may be collected from a third-party source. This may include relatives, or other health service providers. This will only be conducted if the patient has provided consent to the practice to collect their information from a third-party source; or, where it is not reasonable or practical to collect the information directly from the patient. This may include where the patient’s health is potentially at risk and their personal information is needed to provide them with emergency medical treatment.
